That's one of the first things I learnt from my boss as a visa agent, always include the middle name even if they didn't ask for it. Where you have given name, put the two names.

I wonder why people use middle names. I have hated the idea from the onset, I have seen it cause pains to many people. I saw what it did to a friend who had his name's mixed up by a sloppy secretary. I don't do middle names and none of my kids ever will!
